Financial Aid Forms Fall 2019, Spring 2020, and Summer 2020

Please read, complete, and submit your forms carefully, and remember, we’re here to help! As you complete any of these forms, feel free to contact your Success Coach for assistance. Submit only the forms requested on your to-do list in MyInfo or as directed by a Financial Aid staff member.

Appeal for Maximum Timeframe

This form is used to determine eligibility of receiving additional Federal Student Aid according to the timeframe requirement of the Satisfactory Academic Progress Policy.

General Processing Time: 10 to 15 Business Days

  • Submit your Appeal For Maximum Timeframe to Academic Advisement.
  • After its review, Academic Advisement will forward it to Financial Aid for evaluation and a final determination of eligibility.

You will be officially notified of the outcome in your Student Center.

Addendum to Maximum Timeframe

This form is used to make changes to an approved maximum time frame appeal. Changes to your appeal would include adding or removing a class, requesting to repeat a course, or swapping out courses. This form can only be submitted if you already have an approved maximum time frame on file.

General Processing Time: 10 to 15 Business Days

  • You must explain in detail the extenuating circumstances surrounding your need to repeat the class(es) and attach documentation to support your appeal (e.g., medical claims/statement, police report, etc.).
  • Submit your completed addendum to Academic Advisement.
  • After its review, Academic Advisement will forward it to Financial Aid for evaluation and a final determination of eligibility.

You will be officially notified of the outcome in MyInfo.

Appeal for Satisfactory Academic Progress

Students have the right to submit an appeal to address why he or she was unable to meet the standards of the Rio Salado College Satisfactory Academic Progress (SAP) Policy. A determination will be made based on the student's ability to be in good academic standing as outlined in the Satisfactory Academic Progress policies by the end of the student's probation period.


Appeals are accepted if extenuating circumstances existed and official documentation is received for each course failed within a semester (s) during the lifetime enrollment of the student, (e.g. personal injury or illness, serious illness or death within the immediate family, or other circumstances beyond the reasonable control of the student. 


*Note, failed courses due to computer issues, course overload, time management, complexity, etc., are not considered extenuating circumstances. 

SAP Appeal Process:

You are required to explain AND submit official documentation for every failed course or W grade (withdrawn) for each semester during your lifetime enrollment at Rio Salado College.

  • To determine all courses and semesters that contributed to your Unsatisfactory Academic Progress go to:
    my.maricopa.edu and select Student Center (log in using your MEID/password)  go to Academic History click on Maricopa Student Progress

  • Appeals are only reviewed and accepted when the following are submitted:

  • Satisfactory Academic Progress Appeal form is completely filled-out and signed.

  • A statement explaining the extenuating circumstance (s) which contributed to not meeting SAP for each failed course (s).

  • Official documentation that supports the statement for each failed course (s), (e.g. death certificate, medical statements, etc.). 

  • All committee decisions are final, and students will be officially notified of the outcome through their Message Center in the Student Center located at my.maricopa.edu.

*Incomplete appeals will not be evaluated for a determination they will be mailed back to the student.

Appeal for Satisfactory Academic Progress

Consortium Agreement

Submit this form if you intend to take courses at two or more different colleges during the same semester/enrollment period and receive Federal Student Aid based on the combined enrollment at both institutions. Consortium courses must begin and end within a student’s block schedule. Students on consortium who are not attending Rio Salado College as their primary institution are responsible for paying their account balance no later than the first day of class at Rio Salado College. 

Deadline: The consortium agreement must be submitted with all signatures by the start date of the student’s semester block at Rio Salado College (Home institution).

FAFSA Parental Data Override Form

Submit this form if you are a dependent student required to document the lack of parental data on your FAFSA. Students with approved submissions will qualify to be eligible to receive Unsubsidized Stafford Loans.

Federal Loan Discharged Due to Disability

The Financial Aid Office has been notified that you have one or more prior student loans discharged due to total and permanent disability. Submit this form in order to allow the Financial Aid Office to continue its evaluation of your request for Federal Student Aid.

Homeless Independent Request

Submit this form in order to evaluate your current living arrangements and proceed with determination of your eligibility for Federal Student Aid. In order to obtain this form, please visit the Verification Forms and Submission webpage.

Identity and Statement of Educational Purpose Statement

You must submit the ORIGINAL notarized Statement of Educational Purpose (in English or Spanish) provided below. This document must be provided by postal mail only (a faxed or emailed Statement is NOT acceptable and will not be processed).

Request for Dependency Override

Federal Law governing Federal Student Financial Aid assumes that the family has the primary responsibility for meeting the educational costs of students. The Rio Financial Aid Office may determine the dependency status for students who have documented, extreme, and extenuating circumstances where it would be unreasonable or impossible to provide the parent(s)’ information on the FAFSA. If you feel that your circumstances warrant re-evaluation of your dependency status, follow the steps below.

  • Submit this form along with the required documentation to the financial aid office.
  • You will be officially notified of the outcome in MyInfo.
  • You may be asked for additional documentation depending on your individual situation.

Revision Form

Submit this form to cancel your financial aid, request a student loan, and other miscellaneous requests.

School Withdrawal Clearance Letter

If you attended and received financial aid from another college during the 2018-2019 academic year, you must sign this form, and have your prior institution submit its official section. The information on this form determines whether or not you are eligible for aid based on your enrollment periods at the prior institution.

Special Circumstances Appeal

You may submit this form to the financial aid office if your 2016 income information or circumstances have changed considerably and no longer represent your current financial situation for the 2018-19 FAFSA.

NOTE: If a loss of employment has occurred during the 2018 Tax Year and you have not obtained employment, you MUST wait until you have filed your 2018 taxes with the IRS after January 2019 to submit the appeal.

REVIEWABLE CHANGES MAY INCLUDE

  • Reduction of 2017 Income/Benefits
  • Reduction of 2018 Income and Benefits
  • Death of Spouse or Parent

NOTE:

  • If you have a zero Expected Family Contribution, do NOT submit this form, as you have been awarded the allowable maximum of need-based aid.
  • You will be selected for verification upon submitting the Special Circumstance Review form.
  • This form will not be processed if any items are left blank or illegible. Attach all required and supporting documentation to this form.

You will be officially notified of the FINAL committee outcome in MyInfo.
